PS2 70000 Series Melted Lens Syndrome

So what causes this melted lens to occur? We really don't have the answer, but replacing the laser has fixed it in all but 1 case. In one case, the PS2 we replaced the laser had a mod chip -- it melted the new lens we installed [OUCH!].
